All applications MUST be absolutely complete with all doctor information and prices  included. Any application turned in without this info will be declined. No part of this application may be left unanswered or it will be declined. Blessed Arrows cannot cover any cost but the cost of the procedure. No travel expenses will be included. That will be left to the individual family. BOTH Husband and Wife need to fill out their respective parts of this application.

Financial Aid


1. All questionnaires must be absolutely completed - no part of the questionnaire may be 
left unanswered or it will be declined.


2. Must be married - this ministry is for married couples only.


3. Members/actively involved in a Bible believing church body/fellowship.


4. It is mandatory that you share your conviction about the reversal with a pastor or elder in your church. 


5. We need a church name, pastor or elder's name, address and phone number we may contact.

6. We need a testimony of salvation from both husband and wife. (At what point did you realize your need of salvation? Tell us about your surrender to Christ for salvation? When, how old, what church, that type  of thing)

7.We need to know why you believe you need a reversal surgery. Do you believe it was sin? What is you conviction about birth control?

8. Each family must donate something every month to the ministry of Blessed Arrows in order to remain on the waiting list. There is no minimum amount required, as long as you send something. We understand that occasionally things will come up, but you MUST contact us.. If, in a certain month, you are unable to donate then we must hear from you regarding your reasons why. We have, in the past, been very forgiving and understanding when this happens. We will continue to be understanding and we will allow a family to stay on the waiting list, provided they write to us explaining their situation. We cannot write couples each month reminding them of their need to donate. It is each couple's responsibility to stay in touch with us. If we do not receive a donation from you and we have not heard from you about this, then  you will be removed from the waiting list. We are patient and will prayerfully give you time to contact us before we remove you. We will notify you by email if you are removed from the waiting list. You may be placed back on the waiting list per your request; however, you will lose your original spot and will be placed at the bottom of the list. We know that this may sound harsh, but there is already enough work to be done without having to email our waiting list couples when we haven't heard from them for a time. Please understand that this has been the policy at Blessed Arrows since it's inception. We have 
had, unfortunately, to enforce it in the past. We hope to not have to do it in the future.

9. It is required that you have a doctor selected for the questionnaire to be approved. We understand that things may change later, just keep us updated. We need the doc's name, price, and how far from your home you must travel.

10. Blessed Arrows does not help with funds for pre-operative tests, travel, etc... (Blessed Arrows does not help cover expenses prior to surgery). Blessed Arrows has a price cap on how much we will fund towards any reversal: Vasectomies- We will help fund up to $2000 ; Tubals- We will help fund up to $4000.

11. Since the husband is the head and spiritual leader of his family, he must believe this is God's will for your lives. He must agree to the following statement: We acknowledge the Lord Jesus Christ as our Lord and Savior. We trust in His mercies and love, and we seek to honor Him by opening our hearts and our lives up to all of His blessings through proceeding with reversal surgery. We are committed to allowing God to plan our family.

12. If you are funded by Blessed Arrows, you are required to mail copies of all bills pertaining to the surgery/charges that BA helps pay for. This includes funds dispersed to the Doctor, hospital or clinic, etc...

13. Being placed on the waiting list at Blessed Arrows does not guarantee that you will be funded or that you will never be removed.

14. Once accepted to the waiting list, it is your responsibility to notify us of any change of  information on your application (ie; phone number, address, church information....any change whatsoever needs to be reported to the leadership team so that we can keep an updated application for you.) This is your responsibility.

15. Once a couple's fund is completed, they will have 6 months to use the funds for their reversal. If they do not have their reversal within 6 months, the funds will be applied to the next family's fund.

Blessed Arrows is a Christian ministry that was created to support those Christian couples in the Lord who have been lead into repentance for their acts of seeking to prevent God's blessings by obtaining either a vasectomy or a tubal ligation. We believe in obedience first of all. Jesus is Lord over all aspects of our lives, including the number of children He chooses to give. We would certainly enjoy having more children, but we will not do anything unusual to try to "help" the Lord give us some, nor will we do anything to hinder the possibility of conception. This is a non-denominational ministry, that trusts in the Lordship of Christ in all areas of our lives.

It is understood by all participants in this ministry that there is no implied endorsement of any particular physician or procedure. There is no implied or stated guarantee that any expenses will be covered by the ministry. There is no implied guarantee as to the success or relative chances for success of any such reversal surgery. It is understood that surgery is in itself, an event that carries certain known and unknown risks. The persons seeking the reversal surgery accepts all responsibility for the outcomes of any surgery or related medical care, and absolves the Blessed Arrows ministry of any financial responsibilities and will in no way hold the ministry responsible for any and all complications that may result from same procedures.

This information is required! We encourage you to look for a Dr. near you, if you can find one who charges a reasonable fee. We have found that it is possible to save on expenses by traveling to another Dr. who is not too far away. What is the projected expense for the surgery? (Blessed Arrows does not help cover expenses prior to surgery) How far is this physician from where you live?


By signing this questionnaire you are saying that you do not intend to use any form of birth control in the future and that you understand  your name will be permanently added to our funding history page on the website should you receive funding from BA.

Blessed Arrows assumes NO responsibility for ANY bills, Blessed Arrows assumes NO RESPONSIBILITY for injuries or costs resulting from the surgery or travel or consequences thereof. Blessed Arrows gives no warranties as to the success or chance thereof for any procedures, and Blessed Arrows can NOT recommend any particular provider. Blessed Arrows is NOT an insurance company, and there is no implied or actual guarantee that expenses will be met. We acknowledge the Lord Jesus Christ as our Lord and Savior. We are committed to allowing God to plan our family. If you are funded by Blessed Arrows, you are REQUIRED to mail copies of ALL bills pertaining to the surgery/charges that BA helps pay for. This includes funds dispersed to the Doctor, hospital or clinic, etc...

***If you have a balance due for the cost of the surgery, after Blessed Arrows has funded its maximum amount, you will need to send what you owe to complete the surgical cost to the Blessed Arrows Account. No money will be released to the doctors until you have paid the remaining balance due for the surgical cost. If for some reason you choose not to have the surgery, you will receive a 100% refund of all your personal funds that were required above and beyond what BA collected. The funds BA collected for the general amount will be passed on to the next family on the waiting list.***

Please note: Addresses and phone numbers will not be posted to the list or to any of the Blessed Arrows web sites without your permission. However, once Blessed Arrows has funded your surgery, your name will appear on our funding history page.  If at any later time you wish for your name to be removed from the funding history, then we will do that only after you have refunded all monies given to you for your reversal surgery by this ministry.  Otherwise, BA has the right to include your name on the Funding History page.

Blessed Arrows has a price cap on how much we will put towards any reversal:  Vasectomies- We will help up to $2000 Tubals- We will help up to $4000

Please remember the primary purpose this site even exists is to collect donations and FUND these reversal surgeries!!  If that offends you, then this is NOT the place for you.  Collecting donations for the surgeries is what this ministry is about. Make no mistake...IT IS ABOUT THE MONEY to get the surgeries funded
